General Information
- What is meant by my “Termination Date” or “Separation Date”?

Your Termination Date or your Separation Date is the last day you work for Johnson & Johnson.
- What is meant by my “Benefit End Date”?

Your Benefit End Date is the last day on which you are eligible for subsidized health care and insurance coverage. Your Benefit End Date is the date your severance payments end.
- Will I still have access to the For Your Benefit website?

You will continue to have access to the For Your Benefits Website ([email protected]) beyond your Separation Date and Benefit End Date. Representatives can be reached at 1-866-769-8521 Monday through Friday from 9am to 5pm ET.
- How do I change my address after my Separation Date?

To report changes of address please contact Global Employee Service Centre by visiting:

Health Coverages
- When will my medical, dental and/or vision coverage end?

Your Group plan benefits (health/dental etc.) will end on the date indicated in your termination agreement. To access your benefits profile, please log onto the Sun Life Website. If you have not logged on before, you will be required to register first. You may be able to convert some of your coverages to a personal plan. To obtain more information and confirm your eligibility, please refer to the Sun Life’s My Choice Plans brochure for more information.
You must contact Sun Life Financial within 60 days of the date your Company group coverage ends for coverage continuation. You have certain options regarding benefit coverage. All details pertaining to benefits will be provided in your termination letter.Sun Life Financial

Your health benefits continue until the end of your severance period indicated in your termination agreement. For further information, call the Benefit Service Center at 1-866-769-8521 and speak with a representative.
Insurance Coverages
- Will my Life Insurance continue during severance?

Yes, you are eligible to continue your life insurance coverage until your Benefit End Date. After your severance period you maybe eligible to convert some of your Life Insurance Coverages to a personal plan.

You may contact the For Your Benefits Help Centre at any time to access your account and obtain personal information related to your benefits, pension and savings plans.

- Can I continue my Life Insurance after my Benefit End Date?

If you have group life insurance coverage you may be eligible to continue some coverage through Desjardins’ conversion Privilege Plan under the applicable terms. You are also eligible to apply for up to $200,000 (but not exceeding the amount of insurance you had as an active employee) of individual life insurance protection under the terms of Company group plan, without providing proof of good health. You need to contact Desjardin within 31 days of the date your Company group coverage ends for coverage continuation or to apply for an individual policy.

- When does my Accidental Death and Dismemberment Insurance coverage end?

Any accidental death and dismemberment (including 24-Hour Accident Insurance coverage) will contintue until the end of your statutory notice period. Please reivew your My Choice Plans and Desjardin brochure for any options that can be converted to an individual plan.

Retiree Benefits
- Am I eligible for Retiree Medical, Dental and Vision coverage?

To be eligible, you must be at least age 55 on your date of termination and must have completed at least 10 years of continuous Johnson & Johnson Pension Plan Credited Service (either full-time or part-time).
- When can I enroll in Post Retirement Benefits coverage?
-
As a retiree on Severance, you will automatically continue your current active Health Care Plan coverage (Medical, Dental and/or Vision) at the active employee group rate for you and any eligible enrolled dependents until your Benefit End Date. At your Benefit End Date, you may choose to enroll in the Retiree Benefit Plan. You may also elect to defer enrolling in the Retiree Benefit Plan and can elect coverage the first of any month up to age 65.

- When can I enroll in Retiree Dental coverage?

As a retiree on Severance, you will automatically continue your current active Health Care Plan coverage (Medical, Dental and/or Vision) at the active employee group rate for you and any eligible enrolled dependents until your benefit end date. At your Benefit End Date, you will be provided an options package from For Your Benefits to complete your selections. You may also elect to defer enrolling in the Retiree Benefit Plan and can elect coverage the first of any month up to age 65.

- Can I enroll in Retiree Benefits but defer my Pension?

Yes. If you are leaving the Company as a retiree eligible for Retiree Medical, Dental, and Vision coverage, you can elect to take your Retiree Medical, Dental, and/or Vision coverage immediately while deferring Pension payments.
Pension
- When am I eligible to begin my Pension?
-
Employees who are age 55 or older when they leave the company are considered “retired” and have the choice of receiving payments immediately or deferring commencement of payments.
- Can I start my Pension while I am receiving Severance?
-
A calculation of your pension and savings plans benefit will be provided to you within the month preceding the end of your severance payments. If you have any questions about your Company pension and savings plans benefits, please contact the For Your Benefits Help Centre at 1-866-769-8521.
- Does the severance period count toward Pension Vesting Service and Credited Service?

Yes. Accumulation of Pension Vesting Service and Credited Service continues to the end of the notice period.
- What if I am rehired by Johnson & Johnson?

If you are rehired by Johnson & Johnson, all of your previous Credited Service will be restored to you. If you were a Pension Plan member, your membership will resume automatically and you will resume earning Credited Service. If you were not a Plan member, you will become a member when you satisfy the eligibility requirements.

Savings Plan
- Can I still contribute to the Savings Plan during severance?

Pension and savings contributions will continue to be made by you and the Company, as applicable, during the severance payment period after your last day of work.

- What happens to my Savings Plan account after I leave Johnson & Johnson?

If currently enrolled, your participation in the company pension and savings plans continues to the end of the notice period. A comprehensive package outlining your plan options and a full calculation of your pension and savings plans will be provided to you within 30 days following the end of your severance period. If you have any questions regarding your accounts, you may contact the For Your Benefits Help Centre. You may also go online and access your personal profile online at the self-service portal https://jnj.hroffice.com.
- I currently have an outstanding loan in the Savings Plan. How long do I have to pay it back?
